New Patient Information

Thank you for choosing Kidney Disease and Hypertension Consultants as part of your healthcare team. We have multiple
office locations for your convenience. Please call any of our offices to schedule an appointment.

WPlease bring the following items with you to your first appointment :

  • Insurance referrals (if required by insurance company)
  • ID & insurance card(s)
  • Any copay or coinsurance amount you are responsible for (if applicable)
  • A detailed list of all medications you are currently taking including dosages/directions or medication bottles
  • Medical records from your referring physician (if requested)
  • A list of any special questions to discuss with your doctor

Please find a new patient registration packet below for your convenience. Please complete the packet prior to arrival if you are able to. If not, we will help you complete it at the office.

 

Please note: If you need to change the date or time of your appointment, please call at least 24 hours before your scheduled appointment time or as soon as possible if it is an unforeseen situation. Please arrive 15 minutes prior to your appointment time. Please allow 1 hour for your entire visit. We see patients in the order of their appointment times only. In certain unforeseen circumstances, physicians may have to spend more time with a particular patient due to clinical necessity and this may cause some delays with your appointment. We greatly appreciate your understanding and patience.
